June 2011

Dear Peace College Athletes (Returning and First-Year):

Greetings from the Pacer Athletic Department! As the 2011-2012 academic year approaches, we wanted to take this opportunity to inform you of details required for your participation in Peace College Athletics. Please take time with your parent(s)/guardian(s) to read and complete each of the enclosed documents listed below. These forms should be promptly returned to Peace College Athletics. All forms must be completed before you practice - no forms, no practice. The forms can also be accessed at www.gopeacepacers.org, under the Athletic Training tab.

1. Athlete Emergency Medical Information Form

2. Treatment Consent/Authorization for Release of Medical Information Form

3. Acknowledgement of Insurance Form

4. Peace College Sports Participation Health History Form

5. Please include a front and back copy of your current insurance card

Listed below are details of athlete move in and the MANDATORY preseason athlete orientation sessions. All athletes must attend BOTH sessions. You will not be able to compete in your first contest if you do not attend, unless you have prior written approval from the director of athletics.

Athlete Move In

Those athletes living on campus will be allowed to move into their rooms on the dates listed below. Moving in is contingent on approval from the Business Office.

1. All fall sport participants will move in on Sunday, August 21. Specific move in times will be given to you by your coach.

2. All first year winter and spring sport participants will move in on Saturday, August 27.

3. All returning winter and spring sport participants will move in on Sunday, August 28.

Preseason Physicals (MANDATORY)

Mandatory preseason physicals and orthopedic screenings will be conducted for all athletes on August 22 (fall sport participants) and August 30 (winter and spring sport participants) in the Hermann Student Center.

1. Free service provided by the athletic department and Cary Orthopedic and Sports Medicine.

2. Please dress accordingly (practice gear and a tank top under your t-shirt).

3. First-Year students: Please note this physical does not replace the physical required by Peace College Health Services, nor does the physical required by health services take the place of this preseason physical. You must have BOTH to move in and begin practice. You should receive information from health services regarding the college physical and insurance.

4. Fall sport participants physical schedule is as follows (8/22): 2pm, soccer athletes; 3pm, cross country athletes; 3:30pm, volleyball athletes. Your first team practice will follow.

5. Winter and spring sport participants must sign up for your physicals on Tuesday, August 30th, between 2-5:30pm. Please contact head athletic trainer Lyndee Sargent (lsargent@peace.edu) to set up a time.

Athlete Orientation (MANDATORY)

The athletic department will hold an information session in the Hermann Student Center, Monday, August 29th, from 6:30-8:30pm.

1. Complete required NCAA eligibility forms.

2. Review specific requirements outlined in the Peace College Student-Athlete Handbook.

3. Kelly Johnson, director of athletics, and all coaches will be present to assist.

If you have further questions or concerns, please contact Kelly Johnson, director of athletics (919-508-2329, pkjohnsonjr@peace.edu). We look forward to seeing you soon and hope you enjoy the rest of your summer.

Thank you,
